To whom it may concern

• My dentist had determined that I had Periodontitis, and required deep cleaning of teeth under the gumline. The dental clinic checked with Sun Life on-line, as it usually does, but I had to pay the full amount.

• Here is what CDCP/RCSD website says about Periodontal services.

  Canadian Dental Care Plan - What is covered
  Major services > Periodontal services (Periodontitis) cleaning under the gumline treating abscesses bonding for mobile teeth (requires preauthorization) post-surgical evaluations (requires preauthorization) non-surgical gum disease management

• Here is "Co-payments based on adjusted family net income" according to CDCP/RCSD website.

  Lower than $70,000	100% of eligible oral health care service costs will be covered at the CDCP established fees.	0% of the CDCP established fees. You may face additional charges as described below.

• Sun Life is administering the Canadian Dental Care Plan (CDCP) on behalf of the Government of Canada. Please explain why I had to pay the full amount with exactly 0 % from CDCP. Is it possible that wrong family income was applied in calculating my CDCP coverage? Here is my Member ID: 333333-XXXXXXXXXXX

• I received the following S-mail letter on 2025-03-15. I followed the instructions below to renew my CDCP coverage.

  - IMPORTANT NOTICE - Please keep this letter for your records
  We are reaching out to you because your coverage under the Canadian Dental Care Plan ends on June 30, 2025. The Canadian Dental Care Plan requires an annual reassessment to confirm that you still meet the eligibility criteria.

• However, I received the following S-mail letter on 2025-04-10.

  - IMPORTANT NOTICE - Please keep this letter for your records

  Thank you for applying for the Canadian Dental Care Plan. Our records show that you are already enrolled in the plan. After reviewing your new application, the Government of Canada has reassessed your eligibility as follows: You indicated on your application that you have access to employer-sponsored, pension-sponsored, or other private dental insurance. As a result, you are no longer eligible for the Canadian Dental Care Plan and your existing coverage will expire May 4, 2025.

• WTF??? After talking to a human agent, my coverage was restored. Phew!!!
